Patients
If you have completed Home Sleep Testing (at any location) or in-lab sleep testing at a Neurocare location (Newton, or Brockton), you may obtain a copy of your sleep study interpretation report at no cost. Please complete the Medical Records Request form and mail it to Neurocare at 70 Wells Avenue, Newton, MA 02459 or fax it to Neurocare at 617-796-9099. Requests may take up to 30 days to process.
Providers
Copies of In-lab Sleep Study and Home Sleep Study interpretation reports are available to ordering physicians on the Neurocare Physician Portal.
Non-Healthcare Providers
Requests from non-healthcare providers (i.e. Law Office) may be made by completing the Medical Record Request Form. There is a $0.50 per page fee that applies. Please complete our Medical Records Request form signed by the patient and mail it along with a check made payable to Neurocare. Requests may take up to 30 days to process.
